Where Yes is checked, attach a signed explanation with supporting documents. An illegible signature requires a printed name immediately adjacent to it on the form.

Mail to the DOS Office of Notaries, 401 North St., Rm 201, Harrisburg PA 17120 — or submit online at notaries.pa.gov. Include a $42 check or money order payable to Commonwealth of Pennsylvania along with your education course certificate.
